This survey is for all students, grades Pre-K through 12th. Parents will need to assist younger students (Pre-K through 5th grade) in responding to the questions. Each student should submit one survey only. Parents assisting more than one student will have to take one survey for each student.

This is a survey to gather information on how the continuing COVID-19 pandemic has been impacting education on Guam and how efforts to address learning loss and related issues have been effective in providing the needed support and assistance. Please respond to the following questions to help us improve learning for our entire island.
